The drama is based on the manga, but they made Miyabi the protagonist instead of Sanpei, which changed some of the narrative. The manga is more detailed in exploring some of the characters, has more medical cases, etc., but the drama faithfully builds on the world created by the manga to make you feel like you're watching living and breathing real characters by including many subtle hints and references. It feels like the characters would exist and go about their daily business even without the main plot, thanks to the way they've structured and written the adaptation. It's an adaptation done right, fully utilizing the strengths of the different medium.
